You probably noticed that it is expensive to add your dependants (spouse and/or child) on to your School Districts Health Plan. As a member of the AEA and your respective local association you are eligible for discounted rates on a Blue Cross Blue Shield of Arizona individual health insurance plan. You now have a choice of adding your dependents to the Districts plan or applying for an individual plan as an AEA member and reaping substantial savings with more choices on BCBSAZ PPO Plans.
In order to apply, get rates and get benefit information you must do the following:

If your dependents are currently covered by Blue Cross Blue Shield on an individual plan but you are not receiving the membership discount, you do not need to reapply. Please complete the Agent of Record Form and the
Association Discount Form and return them to the above address,
