Dear AEA Member:

You probably noticed that it is expensive to add your dependants on to your School Districts Health Plan.  As a member of the AEA and your respective local association you are eligible for discounted rates on a Blue Cross Blue Shield of Arizona individual health insurance plan.  You now have a choice of adding your dependents to the Districts plan or applying for an individual plan as an AEA member and reaping substantial savings with more choices on BCBSAZ PPO Plans.

In order to apply, get rates and get benefit information you must do the following:

  

• Read Important Application Instructions
• To Apply on-line: Association BCBSAZ Information. Click on Get an Instant quote and apply; complete the information ONLY for those applying for coverage.  If you, as the teacher, are not applying for coverage, you do not include your information on the application.
• Complete application online for speedy electronic submission or print out completed application and mail signed application and $20 check made payable to BCBSAZ (if applicable) and the membership verification form to:

  Be sure to answer "Yes" when asked if you want to share your application with your broker.  This will allow us to assist with the application process.

If your dependents are currently covered by Blue Cross Blue Shield on an individual plan but you are not receiving the membership discount, you do not need to reapply.  Please complete the Agent of Record Form and the  Membership Verification Form and return them to the above address.
